Most designers tend to look at the lack of ux and blame that. Even if this was right before the term was even coined. My husbands father was one of the safety inspectors at 3 mile. There are a lot of bullshit politics in play. But my father in law wouldn’t let a single person continue working or start a project if any of their safety certifications where outdated or had not been obtained yet. Despite the push back he got he was really strict with safety regulations. Even with things so potentially dangerous as nuclear power plants there are always people looking to cut corners. You mention people are quick to blame human error and not UX. Since when is UX NOT a type of human error? Things are not as simple as a control room poorly designed by some engineers. But rarely do we question anything that doesn’t fall into typical ux processes frameworks the usual heuristics. Good article though
